Stroke follow-up has two aims
Preventing a recurrence and managing residual loss of function. That starts with clarifying the cause: carotid Doppler for narrowing in the neck arteries, echocardiography for a cardiac source of clot, and rhythm monitoring for hidden atrial fibrillation. Blood pressure, cholesterol and glucose targets are reviewed, and whether the antithrombotic treatment chosen is the right one is reassessed.
In a significant share of strokes the cause is atrial fibrillation that comes and goes and cannot be caught on a single ECG. If that rhythm is found, treatment changes completely. A 24-hour or longer rhythm recording is therefore a part of follow-up that should not be left out.
What you are asked to prepare
- The discharge summary from the acute admission; it states the stroke type and the treatment given
- Brain CT or MRI images from the acute event, on CD or as digital files
- Your antithrombotic medication: name, dose and when it was started
- Any carotid Doppler, echocardiography and Holter reports
- Home blood pressure readings, ideally covering the last two weeks
- Remaining symptoms: which arm or leg is weak, and any speech, swallowing or vision problems
Process and timing
Because the vascular and cardiac tests are done in different units, appointments spread over several days. With the neurology assessment, carotid Doppler, echocardiography and rhythm recording together, 3 to 5 days is a realistic plan. If rehabilitation is needed, that programme is planned separately.
