Patients often do not remember the seizure. The neurologist asks in detail how it started, how long it lasted, which way the eyes and head turned, and whether confusion followed. The standard investigations are an EEG and a brain MRI using an epilepsy protocol. A routine EEG can be normal, which does not rule out epilepsy. If needed, a sleep-deprived EEG or prolonged video-EEG is requested.
A 20 to 30 second smartphone recording is often more informative than a long verbal description. It can be decisive in distinguishing an epileptic seizure from syncope or a psychogenic non-epileptic seizure.
What you are asked to prepare
- A written account from someone who witnessed a seizure: who, what they saw, how many seconds
- A video of a seizure, if one exists
- A seizure diary: date, time, sleep pattern, any missed medication dose
- Previous EEG and MRI records; the raw traces and images, not only the reports
- Current antiepileptic drugs with doses and, if available, blood level results
Process and timing
The EEG is usually scheduled on a separate day. A sleep-deprived EEG requires preparation from the night before. Taking the clinical assessment, EEG and MRI together, planning 3 to 4 days is realistic. If medication is adjusted, the effect is generally reviewed after 4 to 6 weeks.
