MS is not diagnosed with a single test
The McDonald criteria look for dissemination in time and in space. Contrast-enhanced MRI of the brain and of the cervical and thoracic spinal cord is performed, oligoclonal bands in the cerebrospinal fluid are checked when needed, and evoked potentials may be requested. Disability level is scored with the EDSS.
Dissemination in time is shown by a new lesion appearing relative to an older one. If you hold an MRI from one or two years ago, that comparison can be worth more than a fresh scan on its own and sometimes removes the need for further testing. A report is not enough; the images themselves are required.
What you are asked to prepare
- All previous MRI images, ideally starting from the oldest, on CD or as digital files
- Relapse history: date, symptom, how many days it lasted, whether it resolved
- Steroid courses given and their dates
- Current disease-modifying therapy, start date and any side effects
- Recent full blood count, liver function tests and JCV antibody result if available
Process and timing
Contrast MRI and, if required, a lumbar puncture are scheduled within the same week. A few hours of rest are advised after a lumbar puncture. Planning 4 to 6 days for the whole pathway is realistic. If therapy is changed, the first follow-up MRI is usually requested at month six.
